Four periodontal maintenance visits, exams & X-rays, plus member pricing. For gum-health patients.
Per person, per year. Add your spouse for $149 / yr, each child under 18 for $99 / yr, or a second Posh Perio Plan for $299 / yr.

Treatment credit is toward any treatment of your choice and expires 12 months after issue. Insured referrals earn the credit after completing their first visit. Spouses and children in the same household are family add-ons, not referrals, and do not earn the referral credit.
